Implements circuit breaker as a RoundTripper middleware:
- Closed → Open after consecutive failure threshold
- Open → HalfOpen after configurable duration
- HalfOpen → Closed on success, back to Open on failure
- Per-host tracking via sync.Map for independent endpoint isolation
